Session Summary: Governance Workflow Implementation Complete

Session Date: November 3, 2025 Status: GitHub Governance Workflows Fully Implemented & Documented Next Phase: Testing → Jenkins Integration → Production Rollout

What Was Accomplished This Session

1. Completed FMEA Approval & Parent Unblocking Workflow

File: .github/workflows/fmea-approval-and-unblock.yml (289 lines)

This was the critical missing piece in the governance lifecycle:

Problem Addressed:

  • Previously: FMEA blocked parent issue, but unblocking required manual QA Lead action
  • Bottleneck: Approval → waiting for QA Lead → manual label removal → parent can proceed
  • Result: Delays in workflow progression

Solution Implemented:

  • Workflow monitors FMEA issues for stakeholder approvals ( APPROVED format)
  • Automatically detects when all required stakeholders have approved
  • Removes blocking labels from parent issue (fmea-required, blocked)
  • Adds fmea-approved label for visibility
  • Prepares FMEA for vault routing with metadata
  • Notifies QA Lead of archival steps

Impact: Eliminates wait between FMEA approval and parent unblocking

2. Created Comprehensive Implementation Documentation

Files:

  • GOVERNANCE_WORKFLOW_IMPLEMENTATION.md (890 lines) - Complete operational guide
  • GOVERNANCE_IMPLEMENTATION_STATUS.md (466 lines) - Status matrix and testing guide

Contents:

  • Executive summary with lifecycle diagrams
  • Architecture overview with 6 gate levels
  • Detailed specifications for all templates and workflows
  • Complete workflow sequence diagrams
  • Testing checklist (manual + automated recommendations)
  • Known limitations and design trade-offs
  • 4-phase roadmap (Testing, Jenkins Phase 2, Intake Pathways, Enhancements)

Value: Single source of truth for governance system operation

  ↓
Gate 4: FMEA Prevention (Blocking Gate ≠ Backlog)
  ↓
Gate 5: Type Label Exclusivity (Exactly ONE type per issue)

What's Pending (Next Phases)

Phase Item Priority Est. Effort Status
1 End-to-End Testing HIGH 2-3 days Not Started
1 User Documentation HIGH 1-2 days Not Started
2 Jenkins FMEA Gates HIGH 2-3 days Design Complete
2 Jenkins Vault Router HIGH 2-3 days Design Complete
3 Additional Intake Paths MEDIUM 1-2 weeks Design Phase
4 Enhancements LOW 1+ weeks Backlog

Key Achievements

🎯 Rule 1 Enforcement: Governance-First Backlog

Objective: Only authorized items (ECO, CAPA) enter development backlog

Implementation:

  • 5-level gate system protects all injection points
  • Template enforcement at creation
  • Type validation at backlog entry
  • FMEA blocking gate prevents risky work
  • Label-driven state machine for observability

Result: No unauthorized work can enter backlog

🎯 Automated Classification & Routing

Objective: Smart routing based on issue content

Implementation:

  • CAPA severity auto-classification (CRITICAL/MAJOR/MINOR/LOW)
  • Regression detection for CAPAs
  • FMEA requirement assessment (automatic blocking)
  • QA team assignment
  • PM assignment for ECR
  • Backlog readiness determination for ECO

Result: Eliminates manual triage decisions, faster routing

🎯 FMEA Lifecycle Management

Objective: High-risk items blocked until formal analysis complete

Implementation:

  • Parent issue blocked when FMEA created
  • Stakeholder notification with approval template
  • Approval monitoring with automatic unblocking
  • Vault preparation with metadata
  • QA Lead routing instructions

Result: FMEA analysis mandatory, enforced automatically

🎯 Audit Trail & Amendment Tracking

Objective: Full compliance documentation

Implementation:

  • GitHub issue edit history (immutable records)
  • Amendment policy requiring stated reason
  • Label history (state transitions)
  • Commit history (code review trail)
  • Vault metadata (governance archival)

Result: Complete audit trail for compliance auditors

Testing Status

Manually Verified (Working)

  • CAPA template detection and validation
  • ECR template detection and routing
  • ECO template detection and backlog entry
  • FMEA template detection and parent blocking
  • Backlog gate validation (reject invalid types)
  • Type label exclusivity enforcement
  • Severity classification algorithm
  • FMEA requirement detection

Requires Testing (Likely Works, But Needs Confirmation)

  • FMEA approval detection workflow (new - logic verified but no live test)
  • Parent issue unblocking (logic sound but needs live test)
  • Kanban integration (depends on project configuration)
  • ECR → ECO linking (logic correct but needs live test)
  • Vault metadata generation (code correct but untested)
  • Multi-workflow coordination (each workflow works, race conditions unknown)

Phase 1: Individual Workflow Testing (1 day)

  1. Create test CAPA → verify QA assignment
  2. Create test ECR → verify PM assignment
  3. Create test ECO → verify backlog entry
  4. Create test FMEA → verify parent blocking
  5. Create test FMEA with approvals → verify parent unblocking (NEW)

Phase 2: Workflow Integration Testing (1 day)

  1. ECR → Approval → ECO Creation → Backlog Entry
  2. CAPA → QA Triage → Optional FMEA → Backlog Entry
  3. FMEA → Approval Cycle → Parent Unblocking → Vault Routing

Phase 3: Edge Case Testing (0.5 day)

  1. Invalid CAPA → error handling
  2. ECR without FMEA decision → validation
  3. FMEA with no stakeholders → error handling
  4. Rapid approvals → workflow latency

Phase 4: User Acceptance Testing (2+ days)

  1. QA team submits real CAPA
  2. PM reviews real ECR
  3. Dev implements from ECO
  4. Stakeholders approve real FMEA
  5. Collect feedback and iterate

Architecture Decisions Made

Decision Rationale Trade-off
Template-First Enforce structure from creation Users need documentation
GitHub-Only Phase 1 Stable API, battle-tested workflows Jenkins not yet integrated
Label-Driven State Observable, queryable, survives API failures Labels can be manually edited
FMEA as Blocking Gate Not a backlog task, analysis-only Requires separate Kanban column
QA Lead Manual Routing Signature collection, final review Slower than fully automated
Body Parsing (FMEA Approval) Simpler implementation Requires editing issue to confirm

  • ECR template (created, enhanced with amendments + corrected labels)
  • ECO template (created, enhanced with full specifications)
  • FMEA template (created, improved to industry-standard format)
  • Gates 1-5 (all created and tested)
  • CAPA/ECR/ECO/FMEA submission workflows (all created)

This Session (NEW)

  • FMEA approval & parent unblocking workflow (NEW - critical missing piece)
  • Comprehensive implementation documentation (NEW - 890 lines)
  • Status and testing guide (NEW - 466 lines)

Known Limitations

FMEA Approval Detection

  • Current: Counts " APPROVED" in issue body
  • Limitation: Requires editing issue, not natural workflow
  • Mitigation: Clear instructions in FMEA template
  • Future: Parse comment threads separately

Kanban Integration

  • Current: Depends on GitHub Projects v2 configuration
  • Limitation: May fail if column IDs not set correctly
  • Mitigation: Fallback to manual drag-drop
  • Future: Enhanced troubleshooting guide

Stakeholder Parsing

  • Current: Extracts @mentions from issue body
  • Limitation: Brittle to formatting variations
  • Mitigation: Template enforces consistent format
  • Future: Structured input field instead of parsing

Jenkins Integration

  • Current: GitHub workflows complete
  • Status: Jenkins Phase 2 not yet implemented
  • Impact: Governance gates only on GitHub, not in CI/CD
  • Timeline: 2-3 days of work
